OhioRISE Infographic for Parents Highlights the Basics

Positive Education Program is the Care Management Entity for central Cuyahoga County for Ohio Medicaid’s new specialized managed care program known as OhioRISE, which uses a high-fidelity wraparound approach to help youth who have complex behavioral health needs or multi-system involvement.
If you’re not exactly sure what the previous sentence means, don’t feel bad. There’s actually a lot to unpack in that statement. At PEP, we know the last thing you want to do if your child is in crisis is decipher a service model with unfamiliar clinical terms. That’s why PEP has created an easy-to-understand OhioRISE infographic for parents or caregivers explaining the program in laymen’s terms.

OhioRISE Basics Unpacked
- OhioRISE helps young people with significant behavioral health issues succeed in their schools, homes, and communities.
- Young people who benefit from the program often have multiple needs. These can include needs such as mental health issues, substance abuse, autism, CPS involvement or difficulty with relationships.
- Services wrap around a family and emphasize family voice and choice.
- Care coordination through OhioRISE designs individualized care plans, builds care teams and provides in-home and community resources.
- Families can expect regular communication with their care coordinator, linkage to other services and supports, 24/7 crisis support, and a partner to join you in meetings like those at school and juvenile court.
- Young people must be between age 0-20 to qualify and must be eligible for Medicaid.
- For over 30 years, PEP has provided a similar service through its PEP Connections program.
